Pre-Registration with the Practice

PreRegistrationWelcome to Forestside Medical Practice. Please be aware we have two sites one at Dibden Purlieu and one at Marchwood. If you wish to register with Forestside Medical Practice you can complete our form and medical questionnaire which is included in the Adult Registration Pack and Registration Form within the links below.

Adults Registration Pack

This pack is for anyone aged 16 years and over. You will need to print the forms off, complete and sign them and take them to the surgery of your choice detailed above.


Along with the signed forms your will need to provide 3 forms of identification as follows:

  1. Passport
  2. Driving licence
  3. Birth Certificate
  4. House hold bill in your name less than 3 months old

Please be aware it can take up to 10 days to process your paperwork and register you with our practice. Please also be aware that we will not be able to register your without the completed signed forms.

Once our reception staff have checked your forms, they will need to:

  1. Book you a New Patient Health check with a doctor or nurse depending if you are on medication
  2. Pop you on the scales record your height and pop you on our blood pressure machine.
  3. You will also be given a pot to bring in a urine sample with you for your Health Check.

For children 0-15 you will need to complete the Children's Registration Pack, sign and return this document with their red book and Birth Certificate or Passport.
