We are looking for a Diabetic Nurse Specialist or an Experienced Practice Nurse with an interest in Diabetes willing to take on further training.

The successful candidate will ideally have at least 2 years of experience in primary care practice nursing with a strong ability to identify, manage, and support patients at risk of developing long term conditions & dealing with existing chronic disease management. You will need to be organized, self-motivated and able to prioritize the workload.

Forestside are a patient focused practice covering a population of 12,000 patients. We are looking for a strong team player to join our existing team of 4 nurses, supported by 4 GP Partners, 3 Salaried GPs, 2 Advanced Nurse Practitioners, 1 GP Assistant, 1 HCA & 2 GP Trainees. As a practice we work alongside clinicians and staff from Waterfront & Solent Surgery and The Red & Green Practice based in our Urgent Care Centre within Hythe Hospital and together we form/are the Waterside PCN.

We have an excellent PCN multi-disciplinary team which includes our UCC teams, a first contact physiotherapy practitioner, a pharmacy team, care coordinators, covering health and wellbeing, long-term conditions, postnatal groups, mental health & more. Working together our practice & PCN teams aim to create a positive patient experience.
