Alternative Appointments

TPAL - (The Practice at Lymington)

If we cannot accommodate your needs by having an available appointment at a time when you are able to get to surgery (due to working out of area) then you can contact TPAL to see if they have an available appointment to see a doctor or nurse.  You can contact them on 01590 630545.  There opening times are:-

Monday - Friday 16:00 - 20:00

Saturday - 0800 - 16:00

Sunday - 10:00 - 14:00

They are situated at Lymington Hospital within the Minor Injuries Unit

Pre-Registration with the practice

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. This will only notify us of your intention to register with the practice. You will still need to visit the surgery to complete and sign the form to confirm that the details are correct before we can register you within the practice.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct. This can take up to 3-5 days to process your application.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
