!Letters from NHS England requesting our patients to re-register with another practice!

Some of our patients have received a letter from NHS England (Primary Support England) suggesting their names will be removed from our patient list as their address is outside of our practice boundary (mainly Marchwood patients.) Please be reassured that this request was NOT made by Forestside Medical Practice and that patients are more than welcome to be registered with us at Forestside. We feel this may have been an error on behalf of NHS England which is outside of our control. Please accept our apologies whilst our team are looking into this. Please contact us 02380 844546/871233 if you have received a letter like this so that we can investigate. Your help is greatly appreciated.

New Patient Registration

registrationWe invite all new patients to have a medical check, which includes weight, height and blood pressure and allows us to summarise your medical history whilst waiting for your notes from your previous doctor.  If your are on repeat medication and have on-going problems then this check needs to be done by a doctor. If you are on repeat medication which is stable please bring in your repeat medication slip from your previous practice in order that we may up-date your medication on to our system. If you are not on repeat medication then this check needs to be done by the practice nurse.

As it can take some time for your records to arrive (up to 8 weeks in some cases) this check can be helpful to us.  We are also better able to provide good health care if we know a little about you.

Please also bring a sample of your urine in a sterile bottle for testing.  You will also be given a questionnaire to fill in and an ethnicity form, if you do not return the completed ethnicity form we will assume you do not wish the practice to record this information and record it as refused within your clinical records.  If you would like to undertake this simple health screening, even though you have been with the practice for some time, then please ask the receptionist.
